Ensuring holistic palliative care – you preserve and improve quality of life through sensitive, symptom-oriented care and accompaniment. Individual supply instead of standard solutions – you develop resource-oriented supply concepts, tailored to the needs of each individual patient. Adviserin, supporterin, companion*in – You are competent to assist relatives and cooperation partners, give guidance and support in challenging situations. Specialized care with vision – you carry out complex palliative care measures and symptom-controlling therapies – with medical know-how and human warmth. Building bridges – In close cooperation with the bridge team (SAPV), you will also accompany patients in an outpatient manner in their home. Well networked in the network – you coordinate the service providers involved to guarantee a complete supply. Flexibility with planability – your operations take place from Monday to Friday, supplemented by the takeover of readiness.

Successfully completed vocational training as a health and nursing workerin, elderly caregiverin or comparable qualification Completed training “Palliative Care” with a minimum of 160 hours Knowledge of palliative care and experience in dealing with seriously ill patients (at least two years of practical work) Safe handling of legal requirements, in particular in the drug law, data protection law and the medical device operator regulation Responsibility and expertise in handling sensitive patient data and medical devices physical and psychological resilience as well as team skills and a pronounced social competence secure control of modern communication systems driving licence class 3
