on a mini-job basis for the area outpatiently assisted living in the Nuremberg area.

Your duties

• They accompany, promote and advise clients in their personal environment and in the management of their everyday life according to agreed needs for help in a variety of areas such as e.g. day structure, health, authority matters, social contacts, etc.
• They are responsible for the case in a self-sponsored exchange with relatives, doctors and other important specialist and reference persons from the help system and thus coordinate the support process together with your team colleagues.
• Together with the team, you are responsible for individual assistance planning, the preparation of development reports, the documentation of the provided support services, as well as the implementation and compliance of intra-media quality standards.
• Enable and maintain existing resources and capabilities of the supervised persons and thus promote their quality of life.
• You provide your working time in consultation with the clients, specifying the dates independently taking into account the existing needs.

Your conditions

• You have a completed study of social work (B.A. or Diploma) or as a medical teacher (B.A.)
• They already have experiences in the outpatient social work or in the help of the disabled, or the willingness to acquire the necessary knowledge.
• They can also be self-reflected under difficult conditions in ever-new situations, act safely and independently and make decisions.
• They are loadable, work cooperatively, independently responsible, reliable and structured and convince with a positive basic setting.
• They think resource-oriented and align your actions according to the principles of self-determination and self-employment of clients.
